3. Blotchy rash
One of the symptoms that differentiate strep throat from scarlet fever is the rash that appears with scarlet fever. This rash will be pinkish or red in color and will typically begin on the stomach. The rash will then spread from the stomach to other areas of the body. The blotches on the skin may spread apart or they can join together to form one big blotch. The skin that has the rash will often feel very rough, similar to sandpaper.
